In order to participate in a Georgia Region NSTRA Field Trial you must first register you and your dog with NSTRA and then fill out an official NSTRA entry form which is provided below.  The form can also be found in the file share section of this website. First download the file and save it to your computer. Then fill out the form with the required information. Finally, when you are done filling out the form, you may either send the electronic copy of the form to the designated trial chairman or print it off and mail the hard-copy to the trial chairman.
